Amber Fern, an APRN with the Lincoln Medical Clinic, will be at Tescott
School on Wednesday, May 14, from 9:00am to noon conducting sports
physicals for the 25-26 school year. This includes students wishing to
play Salvation Army Football for grades 4-6 and all JH-HS athletes participating in FB, VB, BB, and/or Track.
It is a flat fee of $25.00. This will not be submitted to insurance. You may pay cash or make a checkout to Lincoln Medical Clinic. If you wish to have your child’s physical
done this day, please complete the KSHSAA physical form dated 3/2024.
Your student will need to pick up the physical form from the school office
if you would like them to participate; please be sure all paperwork is completed with all parent signatures.
Mrs. Fern WILL NOT COMPLETE THE PHYSICAL WITHOUT PAYMENT OR HAVING THE PAPERWORK COMPLETED. Please return the completed physical forms by

If you have any questions please contact the office.
Thank you,
Nurse Becky
